Healthy Beginnings
The Green Mountain School District is committed to providing nutritious meals to the students in the Green Mountain School District. Meals are based on the USDA's nutrition guidelines and include a variety of fresh fruit and vegetable choices, and a variety of chilled non-fat or low-fat milk. With nutritious meals, students are engaged and ready to learn, and helping to establish good eating habits for a lifetime.
Meal Application and Information Form
Free and reduced-price meals are available to all eligible students.
Application forms are also available in the school office. If you have an official AFDC case number or form, just send it to the school office. If you have any questions, contact Carla Whitmire at 360-225-7366 or carla.whitmire@greenmountainschool.us.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

Turn in Your Application as Early as Possible
Applications will be reviewed, and an eligibility determination made within 10 working days of the receipt of the application. Thousands of dollars a year are lost due to eligible families simply not completing and returning these forms. Green Mountain Staff encourage every family to turn in a Free/Reduced Meal Application.
Milk
Regardless of family Free/Reduced/Paid meal status, milk only will be charged at $.50.
Sack Lunch Availability
Sack lunches are available for students going on field trips and on early release days. The lunches will be prepared and based on the USDA’s nutrition guidelines. The sack lunches cost the same as regular lunches. Sack lunches must be ordered three (3) days in advance. There is a check box on field trip forms to order, and a special form is sent home before early release days with the ordering deadline date to return the form.
Charge Policy
All students have a food service account that functions like a debit account. Turn in cash or check payments to the office. When sending cash or check to school, please put money in an envelope with student's name on it. Please make all checks payable to the Green Mountain School District. The student's debit account cannot accommodate charges. Students must have money in their account to cover the cost of a meal or a milk purchase.
Food Accommodations
The Green Mountain food service department will make every effort to accommodate all students with special needs or food allergies. The food service department must be provided a written recommendation or diagnosis from a licensed physician if the child has a disability.
